State Testing Opt Out Information
30-Day Notice for Statewide Tests and Opt-out Form
Parent Request for Opting Out or Exempting Students from Oregon Summative Assessments
- Parents must return completed forms to the school office
- Incomplete or modified forms will not be accepted
- Parents may not write in additional assessments to the state opt-out form
- Forms will be accepted at any time -- parents are encouraged to submit forms by February 1st
If a form is submitted after a student has begun testing, the student will stop testing, but items answered up to that point will be scored and the student will receive a performance level and/or overall score.

Opt-back-in process
If parents change their minds after opting students out of testing and decide to have their students participate, parents must complete an opt-back-in form. Students will be unblocked once a signed form is received.
