Opt-Out Reminder: If you opt out of insurance, you will need to log into OEBB and click the Opt-Out button for medical, decline dental and vision, AND complete the Opt-Out form on the Employee Portal. Once on the Employee Portal, click Tasks in the left menu, then click the Forms tab.

Note about PERS service credit
A PERS member cannot earn service credit for any month in which they are reported as on a leave without pay (OR PFML absences of 11 or more business days in a calendar month are reported as leave without pay).
